Martha Chomjak, 99, of Chillicothe died Tuesday, Jan. 3, 2012, at Heritage Manor in Chillicothe.
Born July 18, 1912, in Clifton, N.J. to Theodore and Anna (Sokol) Phillips, she married John Chomjak on July 19, 1958, in Passaic, N.J. He is deceased.
Surviving are three nephews, Richard (Diane) Noar of Chillicothe, William (Pamela) Noar of Japan and Michael (Carol) Colw of Missoula, Mont.
She was preceded in death by two sisters, Olga and Helen and one niece, Diana Colw.
She worked as a secretary for a heating, ventilation and air conditioning company for about 12 years.
A memorial service will be at 10:30 a.m. Saturday at Bethany Baptist Church in Edwards. The Rev. Lyall Sutton will officiate. Burial will be held at a later date in New Jersey.
In lieu of flowers, memorials may be made to Rescue 33 Ambulance.
